Individualized Case Management

NBRHI coordinates residents social and medical needs through referrals to case managers who will manage the assessment and coordinated services with a multi-disciplinary team of professionals with specialties including behavioral healthcare treatment and mental health services.

In addition, NBRHI and client’s case manager will assist clients with obtaining identification, Social Security, Medicaid and Medicare as well as other vital resources necessary to regain stability and a fresh start.
